| 摘要 | Recently,psychological reactions to cancer have attracted much attention. It has been known that the diagnosis of cancer clearly causes an individual to experience stress related both to the actual symptoms of the disease and to the psychological meaning attached to cancer. The recognition of psychiatric illness, particularly,emotional disturbance by both physicians and nurses is an important component in effective and comprehensive clinical management. The rates of depression and anxiety in cancer patients reported vary in the numerous literature. However,the literature offers little information regarding the incidence,nature,and pathogenesis of emotional disturbances occurring in the gynecologic cancer patients. With this view in mind,the present study was undertaken to determine the prevalence of anxiety and depression in patients with cervical cancer and choriocarcinoma by means of HAD (Hospital Axiety of Depression),BDI (Beck Depression Inventory),and SAS (Self-rating Anxiety Scale). The subjects consisted of 82 patients with cervical cancer and 36 patients with choriocarcinoma who were admitted and visited to the Department of Obstetrics and Gynecology,Kangnam St. Marys Hospital,Seoul,from February,1990 to July,1990. The controls were 63 patients with uterine myoma, and 88 normal controls who visited to the Automated Multiphasic Health System of Kangnam St. Mary's Hospital for the routine physical check-up during the same period. The results were as follows:1. Clinically significant anxiety was found in 28 (34. 2%) patients with cervical cancer and 12 (33. 3%) patients with choriocarcinoma, respectively,using HAD scale. The two cancer groups showed significantly higher frequency of anxiety than uterine myoma 16 (25. 4%) patients and controls 14 (15. 9%) patients (P <0.05). 2. Clinically significant depression was found in 25 (30. 5%) patients with cervical cancer and 11 (30. 6%) patients with choriocarcinoma, respectively,using HAD scale. The two cancer groups also showed significantly higher frequency of depression than uterine myoma (11. 1%) patients and controls (10. 2%) (P <0. 001). 3. With BDI scale,29 (35. 4%) patients with cervical cancer and 18 (50. 0%) patients with choriocarcinoma had clinically significant depression. The two cancer groups showed significantly higher frequency of depression than uterine myoma (23. 8%) patients and controls (21. 6%) (P <0.01). 4. Mean scores of depression of HAD and BDI scale in cervical cancer patients (7.9. +-. 4.4,16.2. +-.11.0) and in choriocarcinoma patients (8. 2. +-. 4. 0,18. 3. +-. 9. 6) were significantly higher than those in uterine myoma patients (6. 7. +-. 3. 4,13. 3. +-. 8. 2) and controls (6. 2. +-. 3. 6,12. 9. +-.10.3) (P <0.01,P <0.01). 5. There were no significant differences in mean scores of anxiety by HAD and SAs, comparing cervical cancer patients and choriocarcinoma patients with uterine myoma patients and controls. 6. Only in patients with choriocarcinoma, there was a significant difference in depressing using BDI among each religion. The patients of Buddhism showed significantly higher scores of depression than those of 3 other religions (P <0.01). In conclusion,these results suggest that a significant number of the gynecologic cancer patients showed clinically significant anxiety and depression. |
